Doctors get help in moving to new model of care

The Kansas Academy of Family Physicians is helping physicians move to a new model of health care delivery with the launch of a patient-centered medical home Web site.

That method of health care delivery is one in which the patient establishes an ongoing relationship with a health care provider who directs comprehensive evidence-based primary and preventive care and coordinates the patient's needs across the health care system.

The Web site was developed with support from a United Methodist Health Ministry Fund grant.
